Been discharged from hospital and need a bit of help to regain your independence….call on us…

CDR Services employs reablement assistance and care workers that provide a range of discharge and reablement services directly to the client.  Our team is trained to handle adults that have been discharged from hospitals and require support during the recovery process including the older people, adults with physically disabilities, people with dementia, people that are frail, mentally ill and terminally ill.

Services shall be provided in the following areas:

  • Facilitation of hospital discharge – during this period our team of professionals and carers will support the service user to maintain or regain the ability to live independently;
  • Preventing avoidable admission through personal and specialist care and
  • Practical help with daily activities, and domestic tasks while service users settle back in and adapt to life out of hospital. These will include personal care, mobility, colostomy care, commode management other household duties.

 

CDR Services promotes independent living for our clients to the greatest possible extent.  Their rights as human beings are observed. The principles of good care are fundamental to our work.
