Referral Process 

CDR Services has a selective discharge support focus on those who are judged most likely to benefit and we offer a general home care for those with longer term healthcare needs.

Assessment Process

Before you leave hospital, we will make an assessment of your condition and another of your home to ensure that it is an environment that supports a safe delivery of care. We ensure that our support focuses on what a person can do or would like to do maintain their independence not only what they cannot do. We also know the situation will change as recovery progresses.

Our service support the aspirations, goals and priorities of each person and don’t offer a ‘one size fits all’ packages of care, instead we tailor our care services to each individual Client’s needs, preferences and experiences.  Personalising care services involves moving away from the traditional ‘gift model’ where we decide what people need and then told them how we’d provide it. We ‘consult’ and ‘involve’ people in decisions about their care, but we are still in charge.

Reablement Assistants need to understand that a one-size fits- all approach will not work in reablement. They need training and support to develop person-centred, outcomes-focused care plans that will help people to achieve their goals.

Personalisation is not just about putting people at the centre of everything we do. It’s about putting them in control of their own lives.
